We will need a copy of your child's birth certificate at the time of registration. If your child has played football with us before, check with us to make sure we still have a copy on file. For all cheerleaders, this is a new requirement, and we will need a copy for any new or returning cheerleaders.

We will need every parent/guardian to sign a form for Consent of Medical Care, Non-Compliance Ruling, Photo Release, and Disclaimer and Signature that all information provided is true and complete.
